Contact Forms on Coach and Location Pages

Coach profile pages and the pages of open CAMPUS Academy locations have a contact form. When you send one, we collect:

• Your first name, last name, email address and phone number
• Whether you are filling in the form as a parent or guardian, or as the golfer. A golfer also tells us whether they are 13 to 17, or 18 or older
• The coach or location you contacted, and the page and button you used to open the form
• The date and time you sent it, and the version of the consent wording you agreed to

When a parent or guardian fills in the form, we collect the parent's or guardian's details only. We do not ask for a child's name, date of birth, exact age or school, and the form has no free-text box.

Automatically Collected Information

• With Academy Locations: We share relevant information with the specific CAMPUS Academy location you have expressed interest in to facilitate your inquiry or registration.
• With the Coach You Contact: If you use the contact form on a coach's profile page, we share your name, email address, phone number, whether you are a parent or guardian or the golfer, and the page you sent the form from with that coach. If you use the contact form on a location page, we share the same details with the coach who handles inquiries for that location. We store your details in our database first, then email them to the coach at the email address we hold for them. Once a coach has claimed their profile, your details also appear in their CAMPUS dashboard. The coach is independent of CAMPUS Academy and may contact you by email, phone call or text message about junior golf lessons.
• Service Providers: We may share information with third-party service providers who assist us in operating our website, conducting our business, or serving you.
• Legal Requirements: We may disclose information when required by law or to protect our rights, privacy, safety, or property.
• Business Transfers: In the event of a merger, acquisition, or sale of assets, your information may be transferred as part of that transaction.
